Printable Forms

For ALL Students (Recreational and Team), you will need the following:

• Student Registration Form

• Open Gym / Special Events Release Form

The registration form is 2 pages long. It is important that both pages be printed and filled out. If you have the option of printing on both sides of a page and want to save some paper, please do so.

Once you have printed and filled out both sides of the form, bring in to your child's first class (or event), or mail it to:
